HealthChoices Program is comprised of two (2) components — a behavioral health benefit and a physical health benefit.  Please note that our agency manages the Behavioral Health Benefit only.

Individuals may apply for coverage through the County Assistance Office.  Most individuals who may be eligible for the Medical Assistance benefit, will be enrolled in the HealthChoices Program, however there are some exceptions such as those in nursing homes.  To apply for Medical Assistance and to see if you will be enrolled in the HealthChoices Program, you may apply in person or online.

Determining Eligibility

To determine if you are eligible for these services, our Managed Care Organization (MCO), Community Care Behavioral Health (CCBH) will utilize Medical Necessity Guidelines to see if you qualify to receive services.  In most cases, Community Care  Behavioral Health will ask your proposed agency to provide information as to why you need these services.  Community Care  Behavioral Health will review your request and make a clinical determination regarding your eligibility:

If you have the HealthChoices benefit, you may be eligible to receive behavioral health services for children and adults.

Find a Provider

If you need additional help in finding local community resources for mental health and/or substance use after you have referenced Community Care's website (link below) or contacted Member Services, Community Care has an additional resource to help.  They can help you and your family by:

  • Listening to your concerns related to mental health and substance use.

  • Providing educational support regarding the complaint and grievance process.

  • Helping you connect to community supports, available services, support groups, peer and family networks and other available county and public resources.
